New Client Paperwork

We are grateful you have chosen Daymark Pastoral Counseling.  Below are four forms that you need for your counseling.  Please print out one copy of each form for yourself (and one for your spouse if applicable).  The General Information and Cancellation Policy forms are to be read and signed. The Personal Profile Form is to be filled out and signed.   Please bring those 3 forms filled out to your appointment and the counselor you are meeting with will get them from you and spend the first part of your appointment looking over the forms before you meet with them.  The last form on this page is written directions to the Daymark office which are for your convenience. The General Information form should answer questions you may have like, "How long is the appointment?” or “What is the cost of the appointment?"   If, after reading the form, you have additional questions please bring them to the counselor's attention when you meet with them for counseling.  Thanks again for your interest in Daymark Pastoral Counseling.
