We are grateful you have chosen to seek counseling through Daymark Pastoral Counseling. Below are three forms that you need to fill out for your counseling. You can either fill the forms out electronically and print them off or print out one copy of each form and fill them out by hand. If you and your spouse are coming for counseling you each need to fill out one full set of forms. 

The General Information and Cancellation Policy forms are to be read and signed. The Personal Profile Form is to be filled out and signed. Please bring those 3 forms filled out to your first appointment and the counselor you are meeting with will get them from you and spend the first part of your appointment looking over the forms before you meet with them. 

The last form on that page is written directions to the Daymark office that are for your convenience. 

The General Information form should answer questions you may have like, "How long is the appointment?” or “What is the cost of the appointment?" If, after reading the form, you have additional questions please bring them to the counselor's attention when you meet with them for counseling.
